Understanding your dental coverage:
Most dental insurance plans are negotiated between insurance companies and employers on behalf of the employees. Some employers offer a variety of benefit options for their employees, with varying expenses to the insured. Four of the most common plan structures are covered below:
- UCR “usual, customary and reasonable” plans
- Direct reimbursement plans
- Preferred provider plans
- Capitation plans (also called dental health maintenance organizations)
UCR plans – Usual, Customary and Reasonable Plans
Most closely resembling the traditional “health insurance plans”, UCR plans allow employees to choose a dentist and pay between 20% and 50% of the service after reaching the predetermined deductible. The insurance company is paid a monthly premium by your employer who, in turn, deducts a membership fee from your paycheck.
Just like traditional health insurance, UCR coverage often varies from insurance company to insurance company and from employer to employer regardless of the provider or employer involved.
Direct Reimbursement Plans
As the name suggests, a Direct Reimbursement Plan directly reimburses you through your employer for a portion of your dental treatment and, in some cases, reimburses the full cost of your dental treatment. Most procedures and treatments are “covered,” (coverage includes any partial reimbursement) and the employee is free to choose his or her dentist. At Designing Smiles, we assist you in determining the amount of coverage your plan offers for each procedure you choose.
Preferred Provider Plans
In these plans, employees are required to select a dental clinic which has agreed to offer reduced-rate service fees in order to be named the preferred and, in some case, the only provider of dental services to the employee.
Capitation/Dental Health Maintenance Plans
Under capitation plans, participating dentists are paid a predetermined fee for each plan. This fee is fixed; regardless of how many times the patient visits the dentist, the type of treatment or the cost of the treatment needed.
Just as many health-care plans are being criticized for lack of appropriate coverage; many dental plans are also considered inadequate in their coverage for modern dental care.
